Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to struck by and crushing hazards: a. On or about July 1, 2016 - at the above addressed jobsite, the employer did not ensure that loads being lifted by a Manitou, model MRT 2540 Plus, telescopic boom rough terrain powered industrial vehicle were restrained by guy lines (tag lines). The employer was erecting precast concrete slabs by rigging them below the forks using wire rope slings.
