Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to crushing hazards while working under unsupported and unsecured equipment: (a) On or about July 5, 2016, in the facility, employee(s) were required to perform maintenance/refurbishing work under an Ellis & Watts mobile medical MRI trailer. Safety pins were not used to keep the trailer's front two hydraulic leg stands/jacks from collapsing in the event of a hydraulic failure. Employee(s) were thereby exposed to struck-by and crushing hazards. (b) On or about July 5, 2016, in the facility, employee(s) were required to perform maintenance/refurbishing work under an Ellis & Watts mobile medical MRI trailer. Rear stationary jack stands (stanchions) were not used to support and secure the trailer's rear against movement or collapse. Employee(s) were thereby exposed to struck-by and crushing hazards. Among other methods, a feasible means of abatement includes, but is not limited to: Follow the trailer manufacturer operator's manual "system set-up" guidelines which address proper use of safety pins and rear jack stands (stanchions) to ensure the trailer is secured and stabilized after initial placement of the trailer in desired location and before employees are required to work under the trailer.
