29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Milark Industries, Inc., located at 128 Rupp Road, Mansfield, OH 44903: On or about April 22, 2016, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the J8 Comco Bender # 100308 (Model KB-50NT; Serial # K-02Z). Employees did not control multiple hazardous energy sources, such as electrical and hydraulic, before employees conducted activities such as, but not limited to, setting up the clamp. J8 Comco Bender # 100308 was not isolated from electrical and hydraulic energy sources prior to engaging in this maintenance activity, which exposed employees to amputation hazards.
