OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. At the M3 facility: On or about June 21, 2016 and time prior thereto, employees were exposed to hazards not limited to entanglement and amputation, due to thread in an operator's hand catching in the feed-roller opening as operators clean and remount package spindles during overply.
