Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: (a) On or about June 18, 2016 for employees engaged in a snubbing operation where well control was not maintained by exposing a hole in a pipe and releasing pressurized well gas and fluid into the work area at the Ryan 14X-09 wellsite approximately eight miles east of Watford City, North Dakota. Abatement Note: Among other methods, one feasible and acceptable abatement method would be to follow API RP 54 (R2013), Section 6.1.1 and maintain well control at all times. One means of controlling a pressurized well is a process called "killing the well". Considerations shall be made to ensure appropriate equipment and materials are on location and operational before work commences.
