Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to stuck-by hazards: (a) On or about June 20, 2016, an employee inadvertently suffered fatal injuries after being struck by a pipe handler boom during drilling operations at the Ross 42-0915H well pad in Ross, North Dakota. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct the hazard would be for the employer to develop and implement a barrier system to prevent employees from accidentally entering the zone of danger during pipe handler boom operations. A physical barrier, such as a guardrail system, would eliminate or mitigate exposure to struck-by hazards as employees at ground level assist with pipe handling activities.
