OSH ACT of 1970 Section (5)(a)(1): the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to caught-in hazards while working in and around belt conveyors: (a) Shredder building, car and truck tire side -- primary and secondary shredder and shaker feed conveyors and stacking conveyors: On or prior to 6/14/16, belt conveyor system(s), including emergency stop controls, were not designed, installed, and maintained to protect employees from caught-in hazards.
