29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about May 18, 2016, employees performed maintenance activities on the Unit 12 South Gas Fan and the employer did not ensure the energy control procedures clearly documented and described the requirements for verifying the electrical energy and residual energy of the mechanical parts of the fan had been controlled.
