Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)Fertilizer Tanks - An employee entered the Ammonium Phosphate-Potassium Chloride Solution tank to clean debris from the tank prior to changing a pump. The employer had not implemented a confined space program to prevent the employee from being exposed to serious harm from a hazardous atmosphere. Among other methods one feasible and acceptable abatement method to correct the hazard would be for the employer to implement a confined space program as specified in ANSI Standard Z117.1 - Confined Space. This violation was noted on or about 4/30/2001.
