Section 88.4 - Code of Iowa (1999) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards of being caught in the space between the work rest and sanding disk as it was in excess of 3/32 of an inch: (a)Tool & Die Area, Wilton Combination Disc/Belt Sander, Model #4202, Serial #30258, Asset #13080 - The sander had a gap between the work rest and the rotating wheel greater than the manufacturer's specification of 3/32". In addition, the brackets securing the work rest to the machine were insufficient to hold the proper clearance. The excess space created an amputation hazard between the rotating disc and work rest while being used by an employee on November 28, 2000. Among other methods, one feasible and acceptable abatement method to correct the hazard is to assure that the gap between the edge of the table and the face of the disc be of sufficient space (3/32 inch) so as not to allow parts to get caught and assure that worn brackets are immediately replaced as referenced in the operating instructions/parts manual for the Wilton, model 4202, Disc, Belt and Combination Disc/Belt Sanders, pg. 13 and the Employer's Work Station Operation Instruction Sheet, item number 10.
