29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated, the person certifying that the evaluation had been performed, the date of the hazard assessment, and, which identified the document as a certification of hazard assessment: On or about May 13, 2016, and at times prior thereto, employees were exposed to fall hazards of greater than 4 feet to the ground level below, while accessing the roof of grain bins and opening the top hatch without the use of fall protection equipment or a complete guardrail system.
