29 CFR 1910.269(a)(4): Existing characteristics and conditions of electric lines and equipment that were related to the safety of the work to be performed was not determined before work on or near the lines or equipment was started. Such characteristics and conditions include, but not limited to: (v) The presence of protective grounds and equipment grounding conductors: (vi) The condition of protective grounds and equipment grounding conductors: On or about on May 10, 2016 at 12281 U.S. 171, Noble, LA, employees were setting up mechanical equipment to install an electrical pole without equipment grounding conductors. The equipment grounding conductors were damaged. The mechanical equipment made contact with the primary electrical power line of 14.4 KV.
