This condition was last observed on and about 05/05/16, at 16051 MO-HWY 47, Marthasville, MO 63357. Inside of the Yard, employee(s) who change out the LPG cylinders belonging to the Caterpillar Fork Truck, m/n GC 40 KS, s/n AT87A01811, are not consistently provided access to suitable hand protection to protect them from frost bite hazards which may occur due to cross threaded propane fittings.
