29 CFR 1910.147(c)(4)(ii): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section: a) 24 Middlesex Avenue, Carteret, NJ 07008. The employer failed to develop and utilize written energy control procedures to be used during the servicing, maintenance of equipment and clearing debris jams, for the Sandvik QE440, Scalping Screener. Incident occurred on or about May 2, 2016.
