29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device. a. R. R. Donnelley, 190 Milliken Drive SE, Hebron, OH 43025; an employee was exposed to ingoing nip points/caught-in hazards during the rewebbing process for Press N-19. The employee's finger was caught in the press rollers during the rewebbing of Press N-19 on March 24, 2016 and amputated. The employer's rewebbing procedure did not meet the same level of protection as lock out/tag out. b. R. R. Donnelley, 190 Milliken Drive SE, Hebron, OH 43025; employees were exposed to ingoing nip points/caught-in hazards during the rewebbing process for Press 24. The employer's rewebbing procedure did not meet the same level of protection as lock out/tag out. c. R. R. Donnelley, 190 Milliken Drive SE, Hebron, OH 43025; employees were exposed to ingoing nip points/caught-in hazards during the rewebbing process for Press N-21. The employer's rewebbing procedure did not meet the same level of protection as lock out/tag out. d. R. R. Donnelley, 190 Milliken Drive SE, Hebron, OH 43025; employees were exposed to ingoing nip points/caught-in hazards during the rewebbing process for Press N-22. The employer's rewebbing procedure did not meet the same level of protection as lock out/tag out. e. R. R. Donnelley, 190 Milliken Drive SE, Hebron, OH 43025; employees were exposed to ingoing nip points and caught-in hazards on Goebel Press N-18 during the rewebbing process. At the time of inspection on April 22, 2016, the employer had posted signs on Goebel Press N-18 that the press would stop/start/speed up/slow down on its own. The employer's rewebbing procedure did not meet the same level of protection as lock out/tag out. f. R. R. Donnelley, 190 Milliken Drive SE, Hebron, OH 43025; employees were exposed to ingoing nip points and caught-in hazards on Goebel Press N-15 during the rewebbing process. At the time of inspection on April 22, 2016, the employer had posted signs on Goebel Press N-15 that the press would continuously "inch", instead of "inching" just once after each employee activation of the "inch" function. The employer's rewebbing procedure did not meet the same level of protection as lock out/tag out.
