29 CFR 1926.502(d)(16)(iv): Personal fall arrest systems, when stopping a fall, did not bring an employee to a complete stop and/or and did not limit maximum deceleration distance to 3.5 feet (1.07 m): a) 699 Route 49 East, Teterboro, NJ Two employees were observed in roofing tasks adjacent to the edge of an overhang-type flat roof, and were exposed to an approximate 11' fall, on or about April 25, 2016. One employee was attached to a rope via a rope grab that was attached to an anchor via a simple knot. The employee was attached to the rope in a fashion that would not decelerate the employee within 3.5 feet. There is no abatement certification or documentation required for this item.
