29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) Dart Container Corporation (D) Line Bagger - The employer did not effectively guard the point of operation on the D line baggers machine head that heats and perforates the plastic film bagger where employees place heat tape, thereby exposing employees to injuries, on or about April 19, 2016.
