Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1): of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were expose to: the hazards of being engulfed, incapacitated or burned by hot steam, water under pressure or pressured gas lines. Site: VAULT #3 WEST WALL OF UNIT #3 ON OR ABOUT 01/19/94, The employer did not provide training to employees that would provide them with the knowledge, that thawing a frozen 3" pipe, that was charged with water pressure , air, water, or steam (approximately 300 degrees) would result in an unsafe practice. Nor was the magnitude of the hazards or injuries which could result from working on such charged systems. Among Other Methods, One Feasible and Acceptable Abatement Method To Correct This Hazard Is: (1)Follow the safety procedures in ANSI Z.244. 1 82 3.2.3 to 3.3.3 American National Standard for personnel protectionlockout/tagout of energy sources minimum safety requirements. (2)Follow the written procedures outlined in the Orange and Rockland Safety Guidelines for Lockout and Tagout procedures dated August 1993 # SG 2020.0, sections 3.0 C and D, 5.1.2.1 b and section 5.4.9.
