29 CFR 1910.135(a)(1): Protective helmets were not worn by employees when working in areas where there was potential for injury to the head from falling objects: On or about April 13, 2016, and at times prior thereto, employees were not required to wear hard hats in areas of the facility where overhead hazards were present, exposing employee to the hazard of being struck in the head by tanks and materials.
