29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: The Andersons, Inc. - Maumee, Ohio: On or about April 4, 2016, the employer did not ensure adequate fall protection for employees accessing the top of railroad boxcars and tank cars in that lanyards designed for restraint only were connected to an active carabiner and used in a fall arrest system. Employees were exposed to approximate ten foot fall hazards.
