Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by molten metal and/or being severely burned from explosions and/or exposure to super heated gases: (a)On or about 12/25/93; foundry area, furnace number 9: Employees used pressurized water stream in an attempt to seal a leak in a recently plugged furnace tap hole which resulted in a super heated gas cloud or a flaming gas cloud enveloping an employee. (b)On or about 1/24/94; foundry area, furnace number 9: Employees were observed using a pressurized water stream to seal a freshly plugged furnace tap hole. Among other methods, one feasible and acceptable abatement method to correct this hazard is to eliminate the use of pressurized water spray; implement a program consisting of safe operating procedures for plugging tap holes, which includes employee training and education; and require the use of refrigerated air to seal minor leaks from the tap hole.
