OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with unexpected release of hazardous energy. At the workplace, an employee conducted hot tap operations on a pressurized gas pipeline. The employer did not evaluate the work to determine if isolation and de-energization of the system was feasible, and/or did not document, implement, and/or follow procedures to control the unexpected release of hazardous energy. This exposed the employee to struck-by and other hazards associated with a high pressure gas release.
