Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazard(s): a) Melting Department, Upper Platform Area - An employee was exposed to the hazard of being struck by the passing overhead crane while attempting to repair the drive motor for the small conveyor on the upper platform area. An active melt shop crane passes within close proximity of this platform. No means were taken to prevent crane travel in this area while the upper platform is occupied. Condition occurred on or about January 13, 1999. Feasible and acceptable methods among others to correct the hazard is to: a. Develop, implement and maintain a written spotter system program to control the crane movement when men are working in the upper coke hopper deck area. b. Implement a crane lock out system to be used when men are working in the upper coke hopper deck area.
