Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, in that; employees were exposed to fall and crushed by hazards when working from an elevated personnel platform, which was being supported by a rough-terrain forklift. (a) Commercial construction site located at 610 E. Main Street, Ellsworth, WI; A work platform used for elevating personnel did not comply with allowable design requirements, including but not limited to the following: i. The work platform extended greater than 10 inches beyond the outside of the rough-terrain forklift wheel base and outriggers, ii. The maximum work load was not indicated on the platform, iii. The weight of empty platform was not indicated on the platform, and iv. The platform did not include guardrails on the front or sides.
