Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard due to being struck by a moving car conveyor or caught between the moving car conveyor and equipment structures when the conveyor is manually started: a) Roto-Pour Area - The roto-pour car conveyor was not provided with an audible and/or visual alarm device to alert employees that the conveyor is about to start, when operated from the remote override control station, during cleaning operations. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1. Install a audible and/or visual alarm device which will be activated when the manual override control station is operated, that can be clearly heard and/or seen at all points along the car conveyor. Reference ANSI B.20.1, Section 5.112(b)(1) 1987. 2. Permit only trained employees to operate car conveyor. Reference ANSI B.20.1, Section 5.12(a) 1987.
