Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to a potential fall hazard while operating the electric manlift in the main elevator due to the lack of a lock on the manlift door. Among other methods, one feasible and acceptable method to correct this hazard would be to install a lock on the manlift car. (b) Employees were exposed to a potential fall hazard while operating the manlift in the north unit due to the lack of a midrail on the manlift car. Among other methods, one feasible and acceptable method to correct this hazard would be to install a midrail/chain on the manlift car. (c) Employees were exposed to a potential fall hazard while operating the manlift in the north unit due to the frayed and damaged rope used for ascending and descending on the manlift. Among other methods, one feasible and acceptable method to correct this hazard would be to install a new rope on the manlift. (d) Employees were exposed to a potential crushing hazard while working near the base of the manlifts in the north unit and in the main elevator due to the lack of guardrails around the base of the manlifts. Among other methods, one feasible and acceptable method to correct this hazard would be to install guardrails around each base.
