Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards while transferring diesel fuel: On or about March 4, 2016, the liquid transfer equipment used to transfer No. 2 Ultra Low Sulfur Diesel fuel from storage tank #62 to a fuel delivery truck exposed employees to the hazards associated with fire. The fuel delivery nozzle was not listed/approved in that it would not automatically shut off thus allowing fuel to continuously dispense until a valve was manually closed, the hose used to transfer the fuel was not fuel rated, and the pump did not have any pressure relief discharge causing the fuel filter to blow out during the incident.
