29 CFR 1910.119(j)(6)(ii): Appropriate checks and inspections were not performed to assure that equipment installed was consistent with design specifications and the manufacturer's instructions. In The Alternative OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or around March 7, 2016 and at times prior thereto at the Pasadena Refining System, Inc. facility in Pasadena, Texas, the employer did not identify, evaluate, and control the hazards involved in operating the Hydrogen and Light Cycle Gas Oil piping (10"-P-0112-Cs) circuit in high-temperature service for an extended length of time without establishing a corrosion rate and monitoring for depletion of wall thickness. AVD- At the Pasadena Refining System, Inc. facility, the employer replaced sections of a pipe segment with new metallurgy and left a section of old piping in place that was forgotten and not reflected in the process safety information and mechanical integrity program.
