29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: The employer is failing to protect employees from cave-in hazards associated with trenches. This was most recently documented on or about 1 March 2016 at the 700 block of Main Street, Hays, Kansas, when the employer did not instruct each employee working in an 11 feet deep trench on the recognition and avoidance of unsafe trench conditions.
