Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the unexpected release of the loader bucket and/or its contents due to rope failure: HEAT TREAT: EMPLOYEES WERE EXPOSED TO THE HAZARD OF BEING STRUCK BY THE LOADER HOIST BUCKET AND/OR ITS CONTENTS AND/OR BEING STRUCK BY THE WIRE ROPE ITSELF WHILE OPERATING THE LOADER HOIST PROVIDED ON THE NORTH AND SOUTH END WHEELABRATORS DUE TO THE IMPROPER INSTALLATION AND MAINTENANCE OF THE WIRE ROPE. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD, AMONG OTHERS, IS TO: 1.FOLLOW THE MANUFACTURER'S GUIDELINES AND/OR ESTABLISHED INDUSTRY GUIDELINES REGARDING THE INSTALLATION OF THE LOADER HOIST COMPONENTS. 2.IMPLEMENT A PROGRAM OF REGULAR, DOCUMENTED INSPECTIONS OF THE LOADER HOIST COMPONENTS AND PERFORM MAINTENANCE AS INDICATED BY THE INSPECTIONS.
