Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck and or crushed by the elevator and to potential fire hazards within the elevator car and they were not properly trained in the recognition of these hazards. Employees using, operating, and working on this elevator were bypassing the inside safety gate and operating the car, opening the outside safety gates by reaching through openings to open the doors from floor levels, and operating the elevator car with the inside car lights dysfunctional and using a cigarette lighter as substitute lighting. One feasible method to correct this hazard is to supply proper training to those employees using the elevator by a qualified person and require that all employees who operate the elevator be licensed operators. A) LOCATION: 123 FIRST ST THOSE EMPLOYEES REQUIRED TO USE, OPERATE, AND WORK ON THE FREIGHT ELEVATOR, WERE NOT PROPERLY TRAINED IN THE RECOGNITION AND AVOIDANCE OF UNSAFE CONDITIONS RELATIVE TO THE ELEVATOR.
