Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the danger of being caught in a pinch point between the shaft end and the vertical I-Beam, injury from a falling roll, and/or being exposed to being located under the roll while paper roll is being mounted into place. (a) PRODUCTION AREA, ANODIZING LINE #4, PAPER UNWIND SECTION, OPERATOR'S SIDE -- EMPLOYEE(S) EXPOSED TO POSSIBLE INJURY FROM PINCH POINT CREATED BY END OF ROLL SHAFT AND I-BEAM, INJURY FROM A FALLING ROLL, AND/OR BEING LOCATED UNDER THE LOAD OF THE PAPER ROLL ITSELF WHILE EMPLOYEE(S) ATTEMPT TO MOUNT PAPER ROLL ONTO THE UNWIND SECTION OF ANODIZING LINE. FEASIBLE AND USEFUL METHOD(S) TO CORRECT THESE POTENTIAL HAZARD(S) WOULD INCLUDE, AT A MINIMUM, THE FOLLOWING: 1. INSTALLATION OF SAFETY LATCH(ES) ON HOOK(S) OF ELECTRIC CHAIN HOIST(S) TO PREVENT HOOK FROM DETACHING FROM CHAIN HOIST DURING A LIFT. 2. INSTITUTION OF OPERATING PROCEDURE(S) WHICH REQUIRE THAT TWO (2) EMPLOYEE(S) MOUNT/DISMOUNT PAPER ROLL(S) ON THE PAPER UNWIND SECTION OF ANODIZING LINE(S) #4 AND #5 WHERE EMPLOYEE(S) ARE REQUIRED TO HANDLE PAPER ROLL(S) BY MEANS OF AN ELECTRIC CHAIN HOIST. 3. THE ELECTRIC CHAIN HOIST SHALL BE OPERATED ONLY IN LOCATIONS THAT WILL PERMIT THE OPERATOR TO BE CLEAR OF THE LOAD OF THE PAPER ROLL AT ALL TIMES ON ANODIZING LINE #4.
