29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) E Lake Dr. Montauk, NY - An employee was operating a Traverse Lift 8040, rough terrain forklift truck, to transport a floating walkway. The floating walkway was held by slings installed on a fork of the rough terrain forklift truck. The slings were installed by the employer without the manufacturer's prior written approval; on, or about, 02/18/2016. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
