29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer is failing to protect employees from potential hazards caused by unexpected machine energization during maintenance activities. This was most recently documented on February 15th 2016, at the Stahl Specialty Company facility located at 1301 Stahl Drive in Warrensburg, Missouri, when employees were performing maintenance activities on the Laempe LFB50_H Sand Core machine, located in the Sand Core Department. The company was unable to produce written machine-specific lockout / tagout procedures for the subject machine, as well as other machines used in production.
