29 CFR 1910.25(d)(1)(x): Ladders were not inspected frequently and those which have developed defects were not withdrawn from service for repair or destruction and tagged or marked as "Dangerous, Do Not Use." A.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, first floor production area, on or about February 10, 2016 and times prior thereto: A Werner WR110-8 Heavy Duty Stepladder was damaged and remained in service for employee use. Damage included, but was not limited to, bent spreader bars, cracked rear rails, displaced rear horizontals, and bent and damaged top bracket. The ladder was not withdrawn from service for repair or destruction, or otherwise tagged and marked as "Dangerous, Do Not Use."
