Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to in that employees were exposed to struck-by hazard created by a falling tree when it was felled using a method that did not control the tree's drop: (a) Jobsite - State Route 156, Avonmore, PA: On or about February 2, 2016, employees attached a pull rope and a bull rope to a dead ash tree to assist in the felling operation. The pull rope and bull rope placed the tree under tension, causing it to split ("barber chair") as an employee was making the back cut. This caused the tree to fall in an uncontrolled manner, striking and fatally injuring the employee who was operating the chain saw.
