29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Aluminum Shapes LLC located at 9000 River Road in Delair NJ: On January 17, 2016, at approximately 9:20 PM, the tip of an employee's right middle finger was cut off while loading dies into the extrusion press. The employer did not report the amputation until January 19, 2016, which was not within the 24 hour reporting time frame.
