29 CFR 1910.178(l)(3)(ii)(C): Powered Industrial Truck (PIV) operator(s) was not adequately trained in load manipulation, stacking, and unstacking for this specific operation: On or about January 13, 2016, employees were exposed to struck by hazards and crushing injuries while the employees were in the process of laying the plating rack on the ground with a Hyster Forklift, Model Number S70FT, Serial Number H187VO3181N, when the plating rack fell and crushed an employee because the powered industrial truck operator had not been trained on the load handling techniques for the task.
