29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about November 11, 2015, the employer did not utilize specific written procedures for equipment such as, but not limited to, the north Amada press brake #72 when employees are performing servicing and/or maintenance activities such as changing dies on the press brake. Employees were exposed to the hazards of amputations and being caught in the machine. Temtrol Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(I), which was contained in OSHA inspection number 933984, citation number 1, item number 1a and was affirmed as a final order on February 4, 2014, with a final abatement date of May 30, 2014, with respect to a workplace located at 15 E. Oklahoma Ave., Okarche, OK 73762.
