29 CFR 1910.119(d)(2)(i)(A): The employer did not include a block flow diagram or simplified process flow diagram as part of process safety information concerning technology of the process. a) On or about 05/16/16 in the SO2 railcar unloading area; employer had a block flow diagram which inaccurately showed the expansion chamber for the SO2 liquid line that had been removed in 2014 exposing employees who operated and maintained the sulfur dioxide rail car system to inhalation of toxic vapors.
