29 CFR 1904.40(a): The employer did not provide an authorized government representative copies of requested records kept under Part 1904 within four (4) business hours: (a) United States Postal Service - 5520 Memorial Avenue N, Stillwater, MN. On or about December 15, 2015, the employer failed to provide copies of OSHA Forms 300 and 300-A or their equivalents, which were requested for calendar years 2012, 2013, 2014, and 2015 to date.
