29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: Location: entiere site On or about: 12/8/15 a) Employees were engaged in demolition and clean up activities. They were exposed to hazards including lack of head and foot protection, lack of fall protection, lack of protection for material dropping areas and lack of demolition engineering survey by a competent person. The employees were not provided with general safety and health training.
