29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Machine number UC450, VersaClipper clip rail machine; On or before December 8, 2015, energy isolation devices were not located and operated in a manner to isolate energy sources while the machine operators performed servicing tasks including but not limited to clearing jammed materials and changing the location of chain dogs on the material feed chain.
