29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a. On or about December 3, 2015, in the storage mezzanine near the skybox and the employee walkway adjacent to this area, the employer did not provide hard hats to protect employees from debris falling from the ceiling. b. On or about December 3, 2015, in the M149-M151 mens locker and the Delivery Bar Code Sorter area, DBCS-1, the employer did not provide hard hats to protect employees from debris falling from the ceiling. c. On or about March 15, 2016, in the Advanced Facer Canceler System area, AFCS-1, the employer did not provide hard hats to protect employees from debris falling from the ceiling.
