29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: (a) On or about December 7, 2015, employees entered the jet zone to the Extruder and were not informed of the entanglement and asphyxiation hazards by the posting of signs identifying the area to be a confined space. (b) On or about November 30, 2015, employees entered the Surge Bin and were not informed of the entanglement and burn hazards by the posting of signs identifying the area to be a confined space. (c) On or about November 24, 2015, employees entered the jet zone to the Extruder and were not informed of the entanglement and asphyxiation hazards by the posting of signs identifying the area to be a confined space.
