On or about November 19, 2015; a. In the manufacturing area, where employees were exposed to caught-in hazards while performing servicing and maintenance work on the puller machine without specific procedures to control hazardous energy. b. In the manufacturing area, where employees were exposed to caught-in hazards while performing servicing and maintenance work on the extruder machine without specific procedures to control hazardous energy c. In the manufacturing area, where employees were exposed to struck-by hazards while performing servicing and maintenance work on the wire puller machine without specific procedures to control hazardous energy. d. In the manufacturing area, where employees were exposed to caught-in hazards while performing servicing and maintenance work on the clip machines without specific procedures to control hazardous energy.
