Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion hazards: (a)CCM Welding, Repair & Fabrication, LLC @ 100 Caspian Ave., Volga, SD 57071: On or about November 5, 2015, the exposing employer did not ensure employees were protected from fire/explosion hazards as a result of working on top of a tank containing a flammable atmosphere. The exposing employer did not identify flammability levels through the use of atmospheric direct reading instrumentation during an initial assessment or continuous monitoring to ensure the material within the tank would not produce flammable vapors. The condition exposed employees to fire/explosion hazards. Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard is to develop and implement specific procedural elements to control explosions in accordance with the National Fire Protection Association (NFPA) 51B-2009 publication, "Standard for Fire Prevention during Welding, Cutting and Other Hot Work." Section 4.2.2 The PAI (Permit Authorizing Individual) shall determine site-specific flammable materials, hazardous processes, or other potential fire hazards that are present or likely to be present in the work location.
