Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Hazard of being burned or killed by an explosion. Rear of building (A) On or about 8/26/92, employees were exposed to the hazard of recieving 3rd degree burns or deadly injuries from an explosion, while performing maintenance work or mixing chemicals in a Pfaudler Pressure Vessel, National Board # 44922, Manufacturer # P387-1214, which was not equipped with any form of pressure release. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS...... (a) 1. Install a safety pressure relief valve on the tank, with the discharge, discharging in an area that is not used as a means of egress or occupation. A safety relief valve is a pressure relief where characterized by rapid opening or pop action, or by opening pressure. 2. Install a rupture disk device on the tank, with the discharge discharging in an area that is not used as a means of egress or occupation. A rupture disk device is a non reclosing pressure relief device actuated by inlet static pressure and designed to function by the bursting of a pressure containing disk.
