OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) 443 - 447 Marlene Way Leola, Pennsylvania - Employees worked from a platform supported on the forks of a Manitou MVT1203L rough terrain forklift, serial number 400818. The platform width was greater than 10 inches beyond the wheel base of the machine on each side, on or about November 16, 2015. b) 443 - 447 Marlene Way Leola, Pennsylvania - Employees worked from a platform supported on the forks of a Manitou MVT1203L rough terrain forklift, serial number 410007. The platform width was greater than 10 inches beyond the wheel base of the machine on each side, on or about November 16, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
