Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)The hoist to the Sewing Department on the 2nd and 3rd floor does not have a hoistway- door locking device to prevent operation if the door is open, and to prevent opening of the door unless the hoist is at the landing. The absence of a hoistway-door locking device places employees at risk of becoming crushed between the hoist and the ground floor, and at risk of falling into the shaft, resulting in broken bones or amputations. One feasible method of correcting this hazard is to install a hoistway-door combination mechanical lock and electrical contact or a hoistway-door interlock as referenced in ASME A17.1, Part I, Hoistways, Hoistway Enclosures, and Related Construction for Hand and Power Passenger and Freight Elevators and Dumbwaiters.
